Families and/or staff are encouraged to complete an online Ages and Stages Questionnaire for children who are identified as possible concerns. We will review these prior to the screening date and it will assist the screening therapist. These will not be available to parents/staff after the screening date unless an individual screening is scheduled with Pediatric Interactions.

After entering this information, the questionnaire may also be downloaded and printed to complete and then sent to Pediatric Interactions prior to the screening appointment

The screening therapist will utilize the ASQ results during the group screening and provide results/recommendations to family.
